<description>My wife and I would like to volunteer and are just within the upper age limit. However, we have both had cancer and survived it so far but would this make a difference to your study?

<description>I am terribly sorry but I have misplaced my questionnaire that you sent me to complete before my visit later this week. Would it be possibile for you to e-mail me a new copy or should I re-schedule my appontment? I am very embarrassed by this and if it is easier for you to cross me of the list of participants, I will be disappointed but, I will understand. For reference in tracking down my participation records, I live at 304 - 1440 Creekside Drive, Vancouver V6J 5B6 - I can be reach at work, 604 891 6575, or at night at home 604 646 0470.

I have given you my work e-mail. My home e-mail is paddygib@telus.net.

Again, I humbly apologize for the inconvenience I have cause you.

Regards
